Introduction
Creating a new questionnaire allows you to collect security, privacy, or compliance information directly from your vendors. When creating a new questionnaire, you can build it from scratch, use a template, or customize it with sections to group related questions and improve flow.
Creating a New Questionnaire
From the left-hand navigation bar, select Questionnaires.
Click the + New Questionnaire button.
Choose one of the available options:
Create a custom questionnaire - a dynamic form you build and manage in Complyance (most commonly used type)
Upload questionnaire file – if you're asking vendors to upload a completed offline file.
Once created, your new questionnaire will open in the drawer view, ready for you to build.
For custom questionnaires enter the key details:
- Questionnaire Name
- Owner (the platform user responsible for the questionnaire, this automatically sets as the user who added the questionnaire initially)
Description – this is shown to the vendor, so use it to explain what the questionnaire is for and how they should complete it
Adding Questions to your Questionnaire
Once your questionnaire is created, navigate to the Questions tab to start building it out.
Select your first question type - Adding the first question will automatically generate your first section.
Multiple Choice – create a list of answers for vendors to choose from (can be single or multi-select)
Text – for open-ended responses
File upload – vendors must upload a file in response
URL – for responses in link format
Email – restricts answers to valid email format
Number - answer must be a number format
Date – vendors must respond with a valid date
Enter the question text and configure the available responses. If you want to make a question required, toggle the “Required” option to ensure vendors must respond to a question before submitting the form.
Apply advanced settings to the question to automatically add submitted files to the vendor documents tab and to link a response to a custom field in the Vendor tab.
To add another question, click + Add New and select the question type.
You can re-order questions by clicking on the six dots to the right of a question and dragging them to a new position, and you can edit or delete questions at any time.
Tip: If you want to change the type of a question after adding it, click the question type in the top-right corner of the question box and select a new type.
Creating Sections in Your Questionnaire
Sections allow you to group related questions, provide additional context, and improve the flow for vendors.
Adding a Section
From the Questions tab, click + Add New > Section.
Enter a section name and (optional) description.
Add questions directly into the section using the + Add New button.
Moving Questions Between Sections
Click the three dots on a question and select Move to section.
Choose the section you want to move it to (or create a new one) and click Confirm.
Changing the Order of Sections
Open the section you want to move.
Click the three dots in the top-right corner and select Move section.
All sections will collapse into a minimized view.
Drag and drop sections using the six dots next to the section name.

Managing Conditional Logic
Complyance allows you to control which questions a vendor sees based on how they answer previous ones. This is known as conditional logic, and it allows you to show or hide entire sections based on vendor responses. This ensures vendors only see questions relevant to them.
To apply logic to a question or a section:
- For a question click the conditional logic icon, or for a section click the three dots next to the section then choose manage conditional logic.
From here:
- Choose whether you want to use AND or OR logic.
- AND means all conditions must be met
- OR means only one condition must be met
- Use the “Show when” section to define your rule:
- Select a triggering question
- Select the response that should cause your chosen question to appear
Once saved, your selected question will only be shown to vendors who meet the condition.
This feature is especially useful for tailoring questionnaires based on vendor type, criticality, or prior certifications.
What Happens Next?
Once your questionnaire is created, you can:
- Add and reorder questions
- Use conditional logic to customise the flow
- Attach supporting files
- Preview the form before sending
When you’re ready, head to the Vendors tab to send it out.
Tip
Use clear, concise language in your questions, and make use of conditional logic to keep questionnaires relevant and efficient for your vendors.
