Introduction
The Questionnaires module works hand-in-hand with the Vendors module to help you assess and manage third-party risk. Whether you're building security questionnaires, sending them out, or tracking responses, this is your central hub for it all.
You’ll find Questionnaires in the left-hand navigation bar of Complyance, just below the Risks section. There are two key modules to be aware of:
- Questionnaires – for creating, managing, and sending questionnaires
- Vendor Management – your central register of third parties
These two modules are designed to work together for a seamless third-party risk management (TPRM) process.
Navigating the Questionnaires Module
When you open the Questionnaires module, you’ll see a table listing all existing questionnaires.
The table includes:
- Type – Whether it’s a platform-built form or a file upload
- Name – The title of the questionnaire
- Owner – The user who created or manages the questionnaire
Click on any questionnaire name to open the Questionnaire Drawer, where you can view and manage all related details.
Managing a Questionnaire
The Questionnaire Drawer has three main tabs: Details, Questions, and Vendors.
Details Tab
This tab shows key information about the questionnaire.

System Fields include:
- Questionnaire Name
- Description (what vendors will see)
- Type
- Owner
Use the Description field to guide your vendor on how to complete the questionnaire, or to provide additional context.
Questions Tab
This tab is where you build and manage the questionnaire content.
You can:
- View and adjust the order of questions
- Add new sections to your questionnaire
- Edit or rewrite question wording
- Add new questions
- Configure conditional logic
- Mark questions as required
- Attach supporting files
Question Types
Click Create a new question to view available types:
- Text – Allows vendors to provide long-form responses
- Multiple Choice – Pre-set answer options; supports single or multi-select
- Date – Requires a date input
- URL – Validates a web address
- Email – Requires an email format
- Number - Requires a number to be inputted
- Date - Requires a date format
- File Upload – Vendor must upload a file as a response
You can also mark questions as required and attach supporting documents to provide context for the vendor.

Advanced Questionnaire Settings
Use Advanced Settings to automate questionnaire-related actions. You can open the advanced settings for any question within a questionnaire by simply clicking the title of the question within the questionnaire.
You can:
- Automatically add submitted files to the vendor’s Documents tab
- Link answers to vendor Custom Fields
Managing Conditional Logic
Conditional logic allows you to show or hide specific questions based on how the vendor responds when they are completing a questionnaire.
To use it:
- Click the three dots next to a question
- Select Manage conditional logic
- Choose between AND or OR logic to build grouped conditions
- Define the triggering question and response
- Select which question should appear when those conditions are met
This allows you to create tailored, dynamic questionnaires based on vendor responses.
Vendors Tab
This tab helps you track questionnaire delivery and completion across vendors.
For each sent questionnaire, you can see:
- Vendor name
- Current status (e.g., Not Sent, Sent, Submitted)
- Recipient email
- Due date
- Submission date (if applicable)
- Which Complyance user sent it
